In accepting this entry I, the participant and agreed, intending to be legally bound, hereby, for myself, my heirs, my executors and asministrators, waive and release any and all rights for damages I may have against parties connected with this WALK, I attest and verify that I am physically fit to undertake the portion of the WALK which I shall do. I grant full permission for CMHA to us any photographs, film, interviews or videotapes of this event for any purpose.
